• ICT Issue Reporting Form Report a problem with a computer, iPad, board or network. SLT and Jasmine IT are notified automatically. Open form
• Rainbow Reward Request Request a Rainbow Reward when a child reaches a house point milestone, or for another special reason. Open form
• Training Absence Request Form Ask for time out of school to attend a course or training event. Open form
• SEND Involvement: Initial Consultation Log an initial SEND concern, or request an assessment such as dyslexia screening, speech and language, or a GDA. Staff only. Open form

Incidents and safeguarding records

Complete these as soon as possible after the incident. They form part of the school's confidential records.

• Bullying and Prejudice Incident Reporting Form Record any bullying or prejudice-related incident involving pupils. Open form
• Violence to Staff Reporting Form Report any physical or verbal violence or aggression towards a member of staff. Open form
• Physical Restraint Recording Form Record any use of physical restraint or reasonable force. Parents or carers must be told the same day where possible. Open form

If you have a safeguarding concern about a child, record it on CPOMS and speak to a Designated Safeguarding Lead straight away. These forms do not replace that.
